Chronic disease management
Ongoing care for diabetes, high blood pressure, heart and lung conditions, thyroid problems and more, led by board-certified internal medicine physicians.
Diabetes, heart disease and high blood pressure are some of the most common health problems in the Central Valley, and they don’t take time off. Our internal medicine physicians are trained to manage several conditions at once and to keep your medicines working together.
A plan you can actually follow
We set goals with you, not for you. Regular check-ins, lab work and medication reviews help us adjust early, before a small problem turns into a hospital stay.
Care that knows the hospital, too
Several of our physicians also work as hospitalists at Fresno hospitals. If you’ve recently been discharged, we can help you understand your new medicines and make sure your follow-up happens on time.

Questions patients ask
How often will I need to come in?
It depends on your condition and how well it’s controlled. Many patients come every three months at first, then less often once things are stable.
Can you take over care after I leave the hospital?
Yes. Bring your discharge papers and medicine list to your first visit, ideally within a week of going home.
Do you manage more than one condition at a time?
Yes. Internal medicine physicians specialize in adults with several conditions and medicines, and in making sure those treatments work together safely.
