Online, by phone or by text. We’ll confirm a time with you.

Fill out the patient packet
Download it, complete it and email it to intake@cchmgfresno.com, or bring it printed.

Bring what you need
Photo ID, your insurance card and a list of the medicines you take.

Arrive 15 minutes early
That gives us time to check you in without rushing.

A photo ID, your insurance card, a list of your medicines (or the bottles), and your completed patient packet.
How do I send my patient packet?
Email the completed packet to intake@cchmgfresno.com before your visit, or bring a printed copy with you.
